Register your
$59.00 Individual Membership During the process of paying by credit card you will be asked for "Billing Information". This will be the name, address, etc. that the CREDIT CARD COMPANY has on record. If the MPD Membership name and address is different than the credit card name and address then complete the "Shipping Information" with the name and address of the person enrolled in our MPD Program. $99.00 Family membership During the process of paying by credit card you will be asked for "Billing Information". This will be the name, address, etc. that the CREDIT CARD COMPANY has on record. If the MPD Membership name and address is different than the credit card name and address then complete the "Shipping Information" with the name and address of the personal enrolled in our MPD Program. |
Once that enrollment is completed your membership goes into effect immediately and with full benefits. If you know which dentist you will be utilizing we will fax your membership confirmation directly to that office. It is not necessary to have pre-selected your dentist and you can change dentists at any time. In about one weeks time you will receive your membership cards by mail.
You may also call 410-682-5517 or 1-800-656-9677 and request an information pack. The information pack will have a copy of the fee schedule, a list of participating dentists and an enrollment form to complete.

along with payment in full of $59.00 for an individual membership or $99.00 for a family membership. Once we receive this information we will enroll you in to the program and send you your membership card(s) by return mail.
